MISSOURI RESIDENTS:
The Missouri Department of Revenue's Tax Administration Bureau has replaced form #455 ( Application for Refund of Motor Fuel Tax ) with forms 4923, 4924, and 4925 .  You can Download Adobe Acrobat Reader to view these forms.

KANSAS RESIDENTS:
The Kansas Department of Revenue Division of Taxation requires that you fill out an Application for Motor Vehicle/Special Fuel Tax Refund Permit ( MF-51) and mail the form plus a $6 application fee to its offices.
